If you have a program like Microsoft Office Excel, there are several templates built into the program. Check out some of the ones that are already built for you and see if any of them are close to what you need. Many times you can find one that is what you need or close too it that doesn't take much modification to make it right for your needs. Sometimes depending on the version there are extra templates on your Microsoft Office CD or can be found on Microsoft website. I guess there are also other spreadsheet programs out there. If you play with the program though and you're somewhat computer literate, spreadsheets are rather simple to create and customize for yourself. For your purposes anyone with a little experience with Excel could build you one in a matter of minutes. I just went and looked at http://office.microsoft.com and there are some templates there.

Go to http://www.google.com and type in something like 'download free spreadsheet' or 'download free weekly spreadsheet' etc.. you get the idea.
If you can't find one, email me and I can make you one that you can print out and then just copy a bunch of them at your local printing shop or Kinko's Office. (or use your own system)
